This form is to submit a recommendation for an applicant to the University of North Texas Master of Science (M.S.) in Speech-Language Pathology Program. In order to complete this form, you will need:

• the applicant’s full name
• the applicant’s email address
• the applicants 8 digit student ID number
• the text of your letter of recommendation (10,000 character limit)

This form will ask you to provide information about yourself (the evaluator) including your name, email, title, institutional affiliation, and relationship to the applicant, along with a few short closed-ended questions about your assessment of the applicant. You will then be asked to provide the text of your recommendation of the applicant. Please note that our recommendation text submissions have a 10,000 character limit; this should be sufficient for most submissions, particularly if headings or signatures are removed.
